HOW TO USE THESE FORMS

1. Right-click the mouse on the link to the form you need. This will open a pop-up menu. From the menu, highlight the option "Save Target As" and click on it.

2. Choose which area of the computer you want to save the form to and click "Save."

3. Go to the area of the computer where you saved the form and open the form. Fill the form out completely. When you're finished save the form again.

4. Log into your e-mail account. Send an e-mail to libraryc@cravencc.edu. Put the name of the form in the "subject" heading, and send us the form as an attachment.

As soon as your form is processed, we will e-mail you your library card number.  Your card will be mailed to you.
